Parent Requirements Form In order to find the perfect employee, we need to understand your requirements in detail. This is the first step in the process. Upon completion of this form, you will need to remit the $150 non-refundable registration fee. (See the Fees page of this website for a complete description of fees.) Upon receipt of the Parent Requirements Form - link this - and the registration fee, we will contact you to schedule a meeting with you in our office or your home to understand your needs in detail and begin the search. At such time we will provide you with the Agency Agreement contract.
